Dragon Dictation Pricing 2026: Professional, Medical, and Free Alternative

Professional v16 is the perpetual desktop license most people refer to as Dragon. It includes the full speech engine, custom vocabulary, command-by-voice macros, and works inside Microsoft Word, Outlook, and most Windows applications. Latest version is v16, optimized for Windows 11 and Office 2021, backwards compatible with Windows 10.

Legal Individual v16 adds legal vocabularies and document formatting tuned for case briefs, contracts, and depositions. The $100 premium over Professional ships ready-to-use legal terminology and skips the custom training step.

Professional Anywhere is the cloud-delivered version of Professional, billed monthly per user. Useful for IT departments that want centralized management instead of installing a perpetual license on each Windows machine.

Dragon Anywhere is the mobile-only product. The app runs on iOS 14 and later or Android, transcribes in the cloud, and is sold as a standalone subscription. It does not bundle with desktop Dragon, even though both share the Dragon brand.

Dragon Medical One targets healthcare. The contract includes a HIPAA Business Associate Agreement, EHR integration with platforms like Epic and Cerner per Nuance product pages, and medical vocabulary across major specialties. Pricing is enterprise-only, quoted per user per year. User reports and analyst notes put typical figures at $1,500 to $2,500.

Dragon for Mac was discontinued in 2018. Nuance has not released a native Mac version since. The only ways to run Dragon Professional on a Mac today are Parallels Desktop or VMware Fusion, both of which require a Windows VM and a Windows license, on top of the Dragon Professional v16 purchase.

The VM path also tends to cost performance. Dragon Professional v16 is CPU intensive, and user reports on the Nuance forum point to slower behavior under virtualization than on bare-metal Windows. Apple Silicon Macs running Windows 11 ARM in Parallels improve the experience but rarely match a native Mac dictation app.

Mac and mixed-platform organizations face three paths. Stack Dragon, Parallels or VMware, and a Windows license per Mac seat. Switch to a Mac-native alternative like Spokenly. The enterprise contract bundles HIPAA Business Associate Agreement, Epic and Cerner integration, medical vocabulary across major specialties, and dedicated support. The trade-off is the price: typical contracts land $1,500 to $2,500 per user per year, before integration services.

Smaller clinics and solo practitioners sometimes look for HIPAA-eligible alternatives that fit a tighter budget. A common path is bring-your-own-key cloud transcription with a provider that signs HIPAA Business Associate Agreements directly. Deepgram, for example, signs BAAs with healthcare customers using their API. In Spokenly's BYOK mode, audio travels from the user's machine straight to the provider under that BAA, and the transcript returns the same way.

Important compliance note: Spokenly does not sign HIPAA Business Associate Agreements directly. The BAA in a BYOK setup is between the healthcare customer and the cloud provider (for example, Deepgram). Procurement teams at larger hospitals usually require a BAA with every vendor in the protected health information path, including the desktop app, which means Spokenly's BYOK approach does not meet that bar by itself. Spokenly also does not integrate with Epic, Cerner, or other EHR platforms today.
